You can print appropriate Health Forms related to your visit and email them to: janet@indigoskymassage.art or bring them with you to first visit.

If you need to fill forms out during visit, please arrive 15 minutes early.

It is not required for a Client to wear a mask, but If a Client feels more comfortable for me wear one, I will. 

I do not file health insurance, but I will provide a letter for you to give to your insurance company with appropriate cpt coding for medical massage.
